Our Summit Program provides Adult and Youth Outpatient services for those experiencing substance use disorders and mental illnesses, includes intensive dual-diagnosis treatment, addiction intervention, trauma-base treatment and services for those with criminal record. Our case managers sit down with each client to help them set goals and then recommend care based on clinical need.

Responsibilities: 

Work with substance use and some mental health clients to meet the needs of the individual served Coordinate with community partner organizations, including continuity of care referrals Link clients with needed resources to promote consistent and effective services / supports in different settings and circumstances Develop therapeutic relationship with clients and their families Maintain accurate records Track court ordered clients and report to the courts Work with incarcerated population in Summit County Other duties as assigned

Qualifications:

2 + years’ experience with a variety of substance use disorders preferred.

Associates degree or higher required. 

Ability to work independently

Utah state Case Manager Certification or the ability to obtain one within 30 days of hire

Excellent boundaries and good judgment required work well as a member of a team

Ability to meet driving standards set by Valley Behavioral Health
